Student Life Forms and Requests

Access the forms you need to support your student group and campus activities. Submit requests for funding, promotional items, chalking reservations, and other Student Life initiatives to help your organization plan and succeed.

Request for Outside Support

If your group is holding a fundraiser or asking for anything to be donated, your group must fill out this form.

Promotional Items

If your group is requesting money for promotional items or t-shirts, you must also complete this form:

Chalking Reservation

If your group would like to write messages on the sidewalk when it is nice outside, you must submit the Chalking Reservation Form

Student Life Funding Request

Student Life Funding Request if your organization seeks additional funds for a project/initiative.

Monthly Senate Involvement

To remain in good standing for Student Life funding, student organization must have
representation at Student Senate each month. This representation can occur in two ways:

  1. At least one organization member attends at least one Student Senate meeting per month
  2. The student leaders of the organization fill out the “Monthly Senate Involvement Form” by the 10th day of the following month. (Example: January’s Monthly Senate Involvement Form must be completed and turned in by February 10th)
